Our services involve professional staff members in developing your individual plan of care, which is based upon identified needs, goals, physician orders for medication, and your personal wishes. Effective pain management is an important part of your care plan.

Psychosocial Care

Psychosocial care is a crucial component of our patient’s individualized care plans. Our psychosocial services are focused on the psychological and emotional well-being of each patient and their family. These services include mechanisms to assist with the patient’s self-esteem, communication, and even coping mechanisms for their terminal diagnosis. Alongside our interdisciplinary team of professionals, our team of social workers are trained and specialize in providing resources and assisting in the creation of individualized care plans to find the appropriate and best care for our patients.

Personal Care and Comfort

We understand that these final stages of life may be difficult, emotionally, spiritually, and physically. Our care team is focused on providing a personalized care plan that ensures quality care and overall comfort. We ensure that all medications are readily available for our patients and provide top-quality medical equipment designed for the patient’s care. Alongside equipment and medications, our care team is also trained to identify any hazards or issues that may result in avoidable discomfort. Our care team is available at all times for any needs or questions regarding the comfort and care of the patient.

Bereavement Support

Here at St. Martin High Desert, we understand the pain and grief you may experience after losing a loved one. During our initial visits, our bereavement team, composed of spiritual counselors, a bereavement coordinator, and volunteers, come together and assess your emotional and spiritual needs, as well as potential hardships you may face after your loved one passes away. Together, the bereavement team and your family work together to assess any potential needs as we follow the patient and their family for up to thirteen months after their passing. We know that losing a loved one is incredibly difficult, so we’re here to offer resources and support that may be useful during your grief.

Spirtual Needs

Our spiritual counselors trained and focused on the overall spiritual care of a patient. We understand that spirituality is a major component of someone’s identity and may desire a spiritual care counselor during their care with our agency. Our team of counselors, including chaplains, are here to service your spiritual needs and offer time, prayer, and other resources during this difficult time.

Physical Care

Our interdisciplinary team's major focus is your comfort and physical care. Our team is composed of physicians, NP's, RN's, LVN's, and CHHA's. Their top priority is to ensure that your care needs are met during the final stages of life. Together, we create a personalized care plan with frequencies that fit the patient’s needs. Whether it’s ensuring all medications are available, providing showers, or even providing top-quality medical supplies, our care team is devoted to your physical care needs.

We Provide Services For Your Specific Needs

Our services are provided by a care team consisting of: Skilled Nurses, Home Health Aides, Medical Social Worker, and Spiritual Counselor, and more. All services are provided under the direction of a physician and/or Medical Director. Arrangements will be made for hospice-approved medications, medical supplies, and equipment, as appropriate. We will work with you to ensure that our services are provided at your convenience to the extent possible.
